Why should I complete this survey?

When did you last tell us about any changes to you, your household and any needs you have?

When we speak with customers, we often find the information we have about them is out of date. This can mean we don’t have the right phone number, we don’t have the most up to date information about who lives with you and whether there have been any significant changes to circumstances. This means we may not be aware of how best we can deliver our services to suit your needs.

Our ‘About you’ survey, is an opportunity for you to update us on anything we might need to know about you and those living with you.

It should take around 5-10 minutes to complete for each person in your house.

What questions will I be asked?

The survey will start by asking for your basic information and contact details. This is so we are able to contact you in the right way if we need to.

We will then ask for more information from you such as your ethnic background, if you require translation services and your gender identity. Some of the questions will be used for equality monitoring purposes and to make sure we are not discriminating against any particular group.

The questions will then ask you about disabilities, health conditions and support needs that you may have. These questions are important to us so that we can consider how we deliver our services. If you have a disability or health condition, you will be asked how it affects you. If you have any support needs, you will be asked about any changes we may need to consider in how we deliver our services.

If you don’t have any disabilities, illnesses or support needs, please still complete the survey and confirm this.

If there are any questions you’d rather not answer, there is a ‘Prefer not to say’ option.

You will then be asked to complete relevant information for everybody else living in your household. This includes all children and any other adults living there as their main place of residence.

What happens next?

Completing this survey does not mean we will be able to implement any changes to our services immediately. Collecting this information is the first stage in learning about the range of potential needs of those living in the homes we manage so we can begin to plan how to use it to adapt our services, where possible.

By completing this survey, you’ll be helping us to get a more accurate picture of our customers. We may contact you to follow up on your answers to some of the questions.
